There is a daily ferry that leaves from the mainland from Blacks Harbour, NB - check the Coastal Transport website for more information. There is also an airport and charter service available, Atlantic Charters. They also do sightseeing flights when you get to Grand Manan. There are also some fishing wharves where you can dock a private vessel.
